By submitting this form and allowing your child to perform in the Summer Show(s) on 20th July 2025, you agree to the following:

1. Filming & Photography
   The show will be recorded and DVDs offered for purchase. You consent to your child appearing in the footage and in photos, which may be shared with parents, on social media, and used for promotional purposes.

2. Costumes
   You agree to supply small, inexpensive items if requested to complement costumes. More info will follow.

3. Chaperones & Assistance
   You consent to approved OBDA chaperones and helpers supervising and assisting your child, including with costume changes.

4. Medical & Emergencies
   You confirm any relevant medical info has been provided. In an emergency, you consent to basic first aid and for emergency services to be contacted if needed.

5. Behaviour & Supervision
   Your child will be supervised by OBDA staff and volunteers and is expected to follow instructions and behave appropriately throughout.

6. Drop-off & Collection
   You agree to follow sign-in/sign-out procedures and ensure your child is dropped off and collected by an authorised adult.

7. Data Protection
   OBDA will store and use your details in line with GDPR, only for show-related purposes.

8. Show Fee
   The show fee (already added to your invoice in April) covers production costs. If your child withdraws after the form has been submitted, the fee may not be refundable.
